5 steps to book your doctor's appointment online
Download the Evergreen Life app
Tap ‘Create an account’ and enter the email address and password that you want to use, or log in with your existing details.
Alternatively, if you don’t have a smartphone, you can also access your GP services via NHS login on the website.

Link to your GP using NHS login
Go to the GP section on the app or website and tap the blue NHS login button to get started. Follow the steps on screen to set up your NHS login. You’ll need to take a photo of a piece of identification to prove your identity. After your identity has been verified, you’ll be linked to your online GP services and GP record, so you’ll be able to book doctors’ appointments.
If you can’t use NHS login for any reason, there’s an alternative way to link to your online GP services using a letter from your GP practice. Follow our help guide here.
Go to the doctors' appointments section
In the GP system section of Evergreen Life, select ‘Appointments’ and tap on the box marked ‘Book an Appointment’ to choose a date and time that suits you. Please note that your GP practice may not offer online appointment booking and some may have disabled it during COVID-19.

See available doctor's appointments to book
If your GP enables appointments, you’ll see a list showing the next available days with appointments at your practice split into Morning and Afternoon. Clicking on any day will show you all of the available doctor’s appointments. If you want to see a particular clinician, you can easily filter the appointments.
Book your doctor's appointment
Simply select the doctor’s appointment you want to book. Some practices may offer appointments with nurses or telephone appointments. You may be asked to enter the reason for the appointment if your GP practice has asked for that information, otherwise, you can simply click ‘Book’ and your appointment is confirmed.

Can't make it?
With the Evergreen Life app, you can cancel your doctor’s appointment at any time – simply view your appointments and click cancel. It’s as easy as that!
