Social prescribing is often referred to as community referral, where GPs and healthcare professionals will refer a patient to a non-clinical community service. It recognises that people’s health is largely determined by environmental, social and economic factors, so seeking more holistic approaches to healthcare is key.

Evergreen Life and PCG Technology solutions have collaborated to develop the next generation of digital social prescribing. By bringing together online GP record access and social care directory technology, we’re providing a cost-effective, efficient way to deliver social prescribing services.

A truly person-centred approach

By integrating social prescribing services/offering within Evergreen Life, we’re promoting a person-centred approach, enabling patients to play a more active role in their own health and wellbeing. We shall offer both self-serve and any assistance or supported intervention that patients need.
